What You Need to Know

Strategic Clinical Outcomes Specialist Responsibilities:

  • Identify and leverage data to reveal areas for improvement in designated outcome(s), at the branch-level, parent-level, region-level and possibly at the system-level
  • Mentor and coach operations and clinician teams toward positive changes in outcome(s); work with an inter-departmental and interdisciplinary approach; use of behavior change principles to motivate transitions
  • Use of AccentCare improvement tools; contribute to the revision and expansion of our toolkit; tailor interventions to the experience at the ground level; co-produce action plans with branch operations; monitor performance of plans
  • Coordinate and lead meetings with your assigned branches to assure compliance, growth and understanding of implementation programs to meet goals
  • Performance of record review and other investigational methods to identify behaviors, assessment information and other meaningful data points that can identify opportunities for improvement and compliance with implementation programs; provide feedback on findings in constructive presentation to clinicians and operations as needed
  • Document and report findings and progress to the Director, CNO and other colleagues and leaders as necessary; present in team and leadership meetings as requested

Qualifications

Strategic Clinical Outcomes Specialist Qualifications:

  • Performance improvement project/program experience important; background in quality will be considered
  • Implementation and change science knowledge a plus
  • A professional certification in good standing related to our work (examples, but not limited to: COS-C, CPHQ, CPPS, CLSSBB, an applicable clinical certification)
  • Working knowledge of Microsoft Office suite
  • Firm working knowledge of improvement and patient safety concepts, practices and procedures and ability to apply in varied situations toward an outcome
  • Comfort and knowledge of reading and assimilating quality and scientific evidence from literature
