Key Responsibilities:
- Patient Care: Provide high-quality nursing care to patients remotely through various telehealth platforms. Conduct assessments, develop care plans, and collaborate with healthcare teams to ensure optimal patient outcomes.
- Telephonic Triage: Conduct telephonic triage assessments to evaluate patient needs, provide guidance on managing health concerns, and determine appropriate levels of care.
- Health Education: Educate patients on various health conditions, treatment plans, medication management, and preventive care measures through virtual consultations and teleconferencing.
- Documentation: Maintain accurate and detailed electronic health records (EHR) by documenting patient interactions, assessments, interventions, and treatment plans in compliance with regulatory standards.
- Remote Monitoring: Utilize remote monitoring technology to track patient vital signs, symptoms, and health indicators. Respond promptly to alerts and escalate concerns as necessary.
- Collaboration: Collaborate effectively with physicians, specialists, and other healthcare professionals to coordinate care, discuss patient progress, and implement holistic treatment plans.
- Compliance: Adhere to all regulatory guidelines, privacy regulations (such as HIPAA), and organizational policies governing remote healthcare delivery.
- Professional Development: Stay abreast of emerging trends, best practices, and advancements in remote nursing practice through ongoing training, continuing education, and professional development activities.
Qualifications:
- Valid RN License: Must hold a current, unrestricted Registered Nurse license in the state of practice.
- Clinical Experience: Minimum of [X] years of clinical experience in a healthcare setting, preferably in areas such as medical-surgical, critical care, or telehealth.
- Telehealth Skills: Proficiency in utilizing telehealth platforms, electronic health records (EHR), and remote monitoring technology.
- Communication: Excellent verbal and written communication skills with the ability to effectively interact with patients, caregivers, and interdisciplinary healthcare teams.
- Critical Thinking: Strong critical thinking, problem-solving, and decision-making abilities to assess patient needs and develop appropriate care plans.
- Compassion: Demonstrated empathy, compassion, and cultural sensitivity in delivering patient-centered care remotely.
- Technology Proficiency: Comfortable with technology, including video conferencing tools, email, and document management systems.
- Time Management: Effective time management skills with the ability to prioritize tasks and manage workload efficiently in a remote work environment.
- Adaptability: Flexible and adaptable to changing healthcare environments, technologies, and patient populations.
Preferred Qualifications:
- Certifications: Additional certifications such as Certified Telehealth Professional (CTP) or Certified Remote Nurse (CRN) are advantageous but not required.
- Specialty Experience: Experience in specific clinical specialties such as pediatrics, geriatrics, mental health, or chronic disease management is a plus.
- Bilingual Skills: Proficiency in languages other than English may be beneficial, particularly in serving diverse patient populations.
