This is a full-time telework position.

At the 24-Hour Nurse Line the Health Coach Consultant utilizes a collaborative process of assessment, planning, implementation and evaluation, to engage, educate, and promote/influence members decisions related to achieving and maintaining optimal health status. Requires an RN with unrestricted active license.

  • The 24-Hour Nurse Line provides a telephone-based information and decision support program to eligible members. The Nurse will be responsible for the following:
  • Facilitates effective health care decision-making.
  • Improves the quality of care.
  • Improves member satisfaction with their health care plan.
  • Facilitates appropriate use of health care resources.
  • Provides up-to-date health care information to help facilitate members’ understanding of their health problems and possible options.
  • Provides assistance by clarifying available options for the member’s particular health problem or issue.
  • Helps members actively and knowledgeably participate with their provider in their own health care decision-making.
  • Promotes healthy lifestyles to help lower personal health risks, preventing illness.
  • Awareness of the member’s available health and wellness programs, and makes referrals to these programs as appropriate.

Required Qualifications

  • Must have active, current and unrestricted RN compact state licensure in state of residence
  • 3+ years of clinical experience required with specific experience in one or more of the following: Disease Management, Emergency Room, Critical Care, Medical/Surgical Care, Oncology
  • 1+ year proficiency in Microsoft Office Suite (including Outlook and Microsoft Word)
  • Must be able to work an 8 hour day shift every other weekend and 4 holidays per year; with earliest start time of 9am EST hours

Preferred Qualifications

  • Managed Care experience preferred.
  • Previous coaching experience preferred
  • Model candidates have telephone triage nursing experience

Education

  • Associate’s Degree required
  • BSN preferred
