Make a difference in patients’ lives, from the comfort of your home! We’re seeking a passionate and organized RN Case Manager to join our dedicated team, primarily working remotely while providing occasional in-person support to patients in San Angelo and surrounding areas.

Responsibilities
Conduct thorough patient assessments and maintain accurate documentation.
Educate and empower patients on available resources, transportation options, and healthcare providers.
Assist patients in finding suitable physicians, therapies, DME (durable medical equipment), and other support services.
Navigate contractual requirements and complete necessary outreaches.
Stay up-to-date on industry regulations and best practices through designated training programs.
Qualifications
2+ years of clinical nursing experience in a clinical, acute, or community setting.
1+ years of proven case management experience in a managed care environment.
Active and valid RN license in good standing within the state of Texas.
