Registered Nurse Care Manager – Remote | WFH

Your Responsibilities

Conduct comprehensive member assessments to understand their needs and goals.

Educate members about available resources, including travel assistance, physicians, therapies, providers, and durable medical equipment (DME).

Assist members in locating the care they need, including physicians, therapists, and other providers.

Complete all required documentation and contractual outreach in a timely manner.

Stay up-to-date on industry trends and complete assigned training modules.

Thriving In This Role

Previous experience in a healthcare setting such as pediatrics, NICU, or home health is a plus.

Strong communication and interpersonal skills are essential to build rapport with members and advocate for their needs.

Excellent organizational and time management skills are required to juggle multiple tasks and meet deadlines.

A collaborative spirit and the ability to work effectively within a team is crucial.
