experienced staff nurse responsible for providing professional, indirect patient nursing care to a diverse patient population, in addition to being accountable for the care outcomes of a clinical population. This position is a clinical leader and scholar who functions as an organizational/system-level expert, aligning activities to achieve strategic initiatives and goals. Subscribing to Benner’s model of clinical practice, the Sr Care Manager position conducts nursing assessments, interventions, treatments, and documents care. Requirements

PEOPLE ESSENTIAL FUNCTIONS

  • Serves as a clinical expert for hospital patients post discharge and in the ambulatory setting, working collaboratively and maintaining active communication with physicians, nurses, office staff and other members of the interprofessional care team to resolve problems that are identified during follow-up phone calls.
  • Drives strategic patient care initiatives by collaborating with department, organization and system leadership by demonstrating effective verbal, non-verbal and written communication, which includes active listening and teaching. Serves as an expert resource for new employees and preceptors.
  • Promotes employee engagement by serving as a change agent, facilitating teamwork with RNs and interprofessional staff. Promotes mindfulness and resilience practices. Effectively engages in and role models peer-to-peer feedback and accountability. Recommends initiates to improve department scores for turnover/retention/engagement.

Service Essential Functions

  • Advocates on patient/family’s behalf to identify and resolve clinical and ethical concerns, utilizing appropriate resources and assists in the identification and improvement to services by proactively identifying and resolving delays and obstacles for the patient.
  • Develops, updates, communicates and audits plan of care, in partnership with the patient, family and interprofessional team across the continuum of care, using the nursing process. Practices in a manner that is congruent with cultural diversity and inclusion principles. Prioritizes care based on the patient’s needs, abilities and cultural preferences. EF)
  • Promotes, develops, and implements effective patient teaching strategies based on learning needs; uses appropriate resources, incorporating planning for care after discharge. Consistently evaluates the patient’s comprehension and adapts teaching methods accordingly.
  • Champions unit/organizational/system activities to improve patient satisfaction and throughput metrics, through peer-to-peer accountability.

Quality/Safety Essential Functions

  • Provides care as the staff nurse leader to complex patients, role modeling the standards of care, as outlined in the Houston Methodist Professional Nursing Practice document. Anticipates/prevents and recognizes/resolves early declines in patient condition and emergent situations. Proactively recognizes, determines a plan of action, and responds to situations, such as Rapid Response/CERTs, and emergency codes. Mentors, and others related to clinical judgment and reasoning including cognitive and implicit bias, using teachable moments to improve safety.
  • Models the standards related to regulatory requirements and professional practice as outlined in the Houston Methodist Professional Nursing Practice document. Maintains awareness of regulatory guidelines and identifies opportunities. Operationalizes and influences the development of service-line/organization/system standards, guidelines, policies, procedures, and protocols.
  • Analyzes unit/organization/system trends to identify gaps in practice or needs to be addressed for future projects.
  • Champions an evidence-based practice change, performance improvement, or research project for a service line/organization/system that meets the requirements of an empirical outcomes (EO) Magnet exemplar to improve quality and safety scores.

Finance Essential Functions

  • Models independent time management effectively and prioritization of daily tasks, minimizing incidental overtime. Coaches and mentors others in time management, throughput, and financial stewardship, including unit/organization capital and operational budgets.
  • Supports department/organizational strategies to achieve financial targets and mentors others to do the same through timely documentation of care, decreasing length of stay, optimizing efficiency, and other areas according to department specifications.

Growth/Innovation Essential Functions

  • Champions new care pathways or service line implementation to improve service to the community. Identifies trends for improvement opportunities or practice changes and supports initiatives using evidence-based practice literature. Researches the change and presents options to shared governance and leadership. Champion for patient devices and innovation projects.
  • Seeks opportunities to identify self-development needs and takes appropriate action. Ensures own career discussions occur with appropriate management. Completes and updates the My Development Plan on an ongoing basis.

Qualifications

EDUCATION

  • Bachelor’s degree or higher from an accredited school of nursing

Work Experience

  • Eight years of clinical nursing experience
  • Five years of care management or related nursing discipline, telehealth preferred
  • Chronic disease management experience preferred

Licenses And Certifications – Required

  • RN – Registered Nurse – Texas State Licensure and/or Compact State Licensure within 90 days or
  • RN-Temp – Registered Nurse – Temporary State Licensure within 90 days and
  • BLS – Basic Life Support (AHA) and
  • Magnet – ANCC Recognized Certification — American Nursing Credentialing Center (ANCC)-recognized certification in an applicable specialty area, i.e. Certified Managed Care Nurse (CMCN), Case Management (Certified Case Manager (CCM), Accredited Case Manager (ACM), Registered Nurse-Board Certified (RN-BC), Certified Diabetes Care and Education Specialist (CDCES), etc.

  • Demonstrates the skills and competencies necessary to safely perform the assigned job, determined through on-going skills, competency assessments, and performance evaluations
  • Sufficient proficiency in speaking, reading, and writing the English language necessary to perform the essential functions of this job, especially with regard to activities impacting patient or employee safety or security

  • Proficient computer skills including data entry, retrieval and report generation; experience documenting in an Electronic Medical Record
  • Knowledge of community resources, health care financial and payer requirements/issues, and eligibility for state, local and federal programs.
  • Exhibits a strong knowledge of the teaching principles of health care information to adult and geriatric populations
